(A) Before follow-up appointment
  1. If you have been asked by the doctor to take any blood test, diabetes test, specimen test, x-ray check or magnetic resonance examination, make sure you attend follow-up consultations at the respective hospital in accordance with the date and time listed in the laboratory Reminder Form/appointment slip;
  2. Please make early arrangements if you need Rehabus Service/Easy-Access Transport Services/barrier-free taxi service;
  3. Elderly patients should be best accompanied by family members when attending an appointment. Please contact the relevant organisations if you need escort service;
  4. If you cannot keep the appointment, notify the respective hospital and make another appointment. Make sure you have sufficient medication to cover the period up to your rescheduled appointment. Ask family members for assistance if you do not know how to count your medication;
  5. There will not be hospital outpatient services when Typhoon Cyclone Signal No. 8 or above is hoisted or Black Rainstorm Warning is in force.
(B) On the day of follow-up appointment
  1. Before you leave home for the appointment, make sure you have your Hong Kong identity card, appointment slip and the test examination record form. Arrive at the hospital before the time scheduled for your appointment;
  2. If you have consulted private doctors or private hospitals or received treatment from them before the follow-up consultation, you should bring along any medication prescribed by them, laboratory test report(s) or X-ray film(s) to the appointment for the doctor to keep a record;
  3. On arrival to the hospital, go to the registration counter, present your appointment slip and pay the consultation fee. Put the reminding portion of appointment slip in the collection box;
  4. Remember to go to the examination room to check your weight, take blood pressure and take a urine test (if required);
  5. Be patient and wait outside the consultation room. The nurse will call your name when it is your turn to see the doctor.
(C) After consultation
  1. Remember to make an appointment for your next consultation at the registration counter;
  2. Pick up an appropriate apparatus and take blood tests at the blood collection counter;
  3. If you are required to take an X-ray examination before the next follow-up consultation, go to the X-ray Department to make an appointment;
  4. Produce your prescription sheet at the dispensary and collect your medication.
(D) Points to note after consultation
  1. You must take medicine according to the schedule prescribed and dosage given;
  2. If you develop a rash or any discomfort after taking the medicine, seek treatment at the Accident & Emergency Department of a hospital immediately and inform the medical staff.
