(A) Notes on hospital admission

  1. Arrive at the hospital on time;
  2. Present your Hong Kong identity card and the admission slip at the Shroff and complete the admission procedures;
  3. If needed, bring along the relevant examination report(s) and the prescribed medicine that you are taking to the appointment for the doctor’s reference;
  4. Do not bring valuables or large sums of money with you to the hospital;
  5. Notify the respective hospital if you cannot keep the appointment;
  6. Surgery will be postponed when Typhoon Cyclone Signal No. 8 or above is hoisted or Black Rainstorm Warning is inforce. The respective hospital will inform individual patients of the new admission arrangements.
(B) Notes on hospital discharge
Before hospital discharge
Listen carefully when the nurse explains the following:
  • The date of examination and follow-up consultation;
  • If needed, ask family members to contact the departments you have been referred to by the doctor, such as the Community Rehabilitation Service or the Medical Social Work Unit;
  • Ask the nurse if you need to buy or lease medical items
    - Examples of items to buy: ostomy bags, nebulizers, dairy products, and walking aids;
    - Examples of items to lease: oxygen concentrators, wheelchairs and walkers;
  • You can apply for the Easy-Access Transport Services or arrange your own transportation when leaving the hospital.
Hospital discharge
  • Elderly patients should check if they have packed all personal belongings such as dentures, glasses or medication they need to take;
  • The nurse will give you a hospital admission slip in brief, as well as information about your follow-up consultation and items to be purchased or leased;
  • Make sure you know when and how to take medication, the dosage and how to store the medicine;
  • If you are allergic to any medicine, you should always carry an allergy card with you and inform the doctor;
  • Complete the hospital discharge procedures.
Follow-up consultation
  • Confirm the date of the follow-up consultation. Remember to bring along your Hong Kong identity card, the appointment slip and your hospital admission slip;
  • Bring with you the medicine that you are currently taking;
  • If you need to change your appointment to an earlier date, call the Specialty Out-patient Clinic of the respective hospital for registration;
  • In case your health condition gets worse, call ambulance service and go to the Accidents & Emergency Department of the nearest hospital immediately. Bring along your hospital admission slip and give it to the doctor for his reference.
